The error code you provided, CE-34878-0, doesn't pinpoint any specific problem. It's an error code that simply indicates a crash. You may have to try troubleshooting.
PlayStation recommends doing the following. Try these and see if the problem persists:
Make sure the PS4 is updated
Unplug your PS4 camera and deactivate face recognition, if applicable.
Reinstall the original HDD, if previously replaced
Which update was installing? An update to an app in your library, or a software update?
The update of the game could have a bug, if it only occurs in one game. You could report it to the developers, and they could fix it.
Based on your completed guides, it seems you have recently replaced the HDD. Try reinstalling the original, as PlayStation suggests.

the old one broke i heard clicks from the hdd
Then you need to replace it to get it fixed!
@dan0 replacing it is what caused all of this.
Ah, I thought an update caused this. @ryanklothk25632, did you actually already replaced the harddisk for a new one, or working one? If yes, were you able to install the software completely?
Yes I did and I still have the reinstall usb

Hi!
Do you have an error code?
Does the PS4 make a lot of noise?
Does it happen with disc games and downloaded games?
Could be the HDD, could also be the heat..

Okay, that always a bad thing. I would try to check if it's updated for sure. If yes, you can reinstall the software completely and see if that works. If not, try replacing the HDD for a new (or known working) one and install the software on it.
