My iPad was charging as usual, when I hear crackling/sizzling noises coming from the area and smell smoke. I quickly unplug the iPad and discover that the adapter is charred on the inside. What went wrong?

If you have a near by Apple store you might want to take it to them. Clearly something internally failed. What is worrisome here is the fuse links inside failed to blow.
Do you remember where you got it? If you still have your paperwork bring that with you as well.
