First : take a wire continuity tester, and give each wire color it's place on the jack. Here are the colors for this particular model. Yours can be different, there is no standard. If you don't have a tester, use a tiny battery and LED with a resistor (about 1k) in series (if not, short-circuit will burn your wire).
