where should the flat washer be placed? It fell out before I could see where it was located

flat washers normally go on screws. If the screw has a lock washer, they go on after the lock washer.
If it is just a spacer, then you need an exploded view, that you may be able to find on a parts supplier web site.

Looking at the parts diagram there are no washers used. There are o-rings however.
You didn't mean the diffusion plate by any chance?
