In addition to @jayeff's excellent information, if you prefer a written, step by step guide, the guys over at SOSav in France have one of those. Like Jayeff's video, it's also in French, but if you use it with a browser that does translation like Chrome, it is quite usable no matter what your native language is.

They use a hot air gun to soften adhesives, but with a little more patience it can be done with a hair dryer. Note that the back side of the phone is glass, so take care removing it; I found it all too easy to break the cover when I did a Samsung. Use your heat gun / hair dryer frequently as you work and reapply heat if it resists you.
The only other thing to mention is you'll probably need a sheet of precut adhesive to glue the rear case back on, so be sure to order one when you order your screen. Clean off the old adhesive well using 90% or higher concentration isopropyl (rubbing) alchohol.
