You will most likely have to clean or replace the earpiece speaker. That's the one at the top of the phone that you listen to when you hold the phone to your ear. On speakerphone mode, it uses the lower speaker instead, which is obviously working.

Without knowing what model of iPhone you have we can't give you any more specific information, but be aware that on an iPhone X or later model, that speaker will be part of an assembly that will result in the loss of Face ID if you replace it. There are workarounds, but again we would have to know which one you have.
