Repair information for your particular phone seems to be a little hard to come by, but based on similar Motorola models and listings for parts for sale, it appears there are at least three different approaches you could take.
The first, and simplest, would be to just replace the entire back of the phone. Not knowing where you're located, it's hard to give you specifics on where to get one, but here's an example from the UK; you should be able to find others that would be more appropriate for you.
As best I can tell, removal of the rear cover is fairly straightforward; generally I would assume you can just use heat to soften the adhesive then go around the edge with a pry tool to separate the adhesive, at which point the cover should come off. You might be able to find instructions with a search on YouTube.
The second method would be to replace the bezel that holds the camera lenses. Here's an example that should be available throughout most of the world. Although it is supposed to match the color of the phone, this particular one is only available in black, but I'm sure a more extensive search would probably pull up more colors.
Again, I only have pictures to go by, but I believe once you have the back off the phone, you should be able to replace that assembly by prying it out and regluing the new one in its place.
The final method I have for you is to replace just the glass, which I'm guess was your original intent. For that, you don't need to remove the rear cover, but if you want the best chance of a successful repair, that's the way I would do it. Here's an example of the glass available for purchase.
The glass is simply held within the bezel with adhesive, so replacing it is simply a matter of prying the broken pieces out, cleaning it well, and gluing the new glass back in. You do have to be careful not to get liquids or glass pieces inside the cover in the process of removing the old glass, but if you're careful there shouldn't be any problems. Heat may help loosen the glass, and 90% or higher concentration isopropyl (rubbing) alcohol will clean off the adhesive.
If you really want to play it safe, removing the rear cover first will eliminate the possibility of getting glass shards in the camera, and makes removing the glass simpler, as you can push it out from the rear side. It is more work, of course, and you will have to replace the adhesive holding the rear cover in order to reseal it.
Anyway, those are all the considerations I can think of, hopefully one of those will get your repair going.
Should you settle on a method and succeed with your repair, might I suggest you take lots of pictures as you work and once you're finished, use that to create your own iFixit guide for those who come after you with the same problem.
