Currently ifixit only has the battery in stock but not the LCD display although it seems to be readily available online. Search for iPod Nano Gen 4 LCD screen to find suppliers that suit you best.
Here's the ifixit iPod Nano 4th Generation repair guide that may also help with the above.
I couldn't find any suppliers of the charge port. You may have better luck.
Here's a previous answer on ifixit regarding the charge port that may help.
Here's a video that shows what's involved in replacing it. I realize that it is not the same device but the procedure for removing/replacing the port from the board is the same.
