internal hd not booting - logical volume os x extended (not present)

Hi All -

I'm having issue with my MBP,

I'm trying to install a fresh macOS Sierra on my MBP

My problems are:

Internal HD

-Media Disk (Present)

-Logical Volume (NOT Present)

I created a logical volume disk to boot off of , but it can't find "disk destinations"

im not sure what to do , to get a clean reboot..

please help ASAP
