Well, that only leaves the front facing microphone is part of the earpiece [product|IF240-035] if you look at the image of the part you’ll see two gasketed openings one is the earpiece speaker the other is the microphone.

It’s likely the microphone is bad. Do you still have AppleCare? If you do this would be covered under it.
