***OK, lets look at that screw!*** It's likely you cross threaded it as these screws are at an angle! I would take it out fully and rethread it in this time take note of the surface angle.
A simple trick here is take a business card draw a line spitting the long side into two equal parts now take the card place the line above the screw hole, place it so at the screw hole the cards edge is 90º to the hole shaft now follow this trajectory of the line with the screw. That should get you flush!
