while look at this video, at 11:18, m.2 ssd slot is on next to the usb ports, which is the end of slim edge of backside of imac case.
I found some m.2 ssd heatsink that has z-height under 4~5mm. Can I attach thease heatsink?

@danj Um... What kind of issue? I found many successful reviews of iMac users who has install m.2 ssd with sintech adapter. I'm considering buy 2019 iMac 27inch and 1tb ssd. But apple's original proprietary ssd is too much expensive. At the article you gave to me (ultimate guide blabla), the price of gen 5 1tb ssd is $898. Let me see the price of samsung 970 pro 1tb.... $332? Game is over.
Ok then, if there is not a critical issue, I'll just buy samsung 970 pro or evo. I'm not milionare.
Besides, it is much clever to just buy iMac with ssd cto option. Why should I spend my time to spend more money by buying more expensive ssd with taking the risk of breaking the apple care? You know, it is nonsense.
(I've already have original adhesive strip tape which bought from mac repair company. They say there are so many unqualified iMac strip tape that sells in the online market. They said they are using that tape at their repair job, and they've done self testing compare with other tapes. So I trusted it.)
I found another ifixit link. Oh there is your comment, hi @danj

Now I know your side of thinking.
Thank you but I'll choose by myself.
