If your phone was made in 2016, it's almost certainly in need of a new battery. As batteries age, they lose the ability to hold a full charge. It's like a glass of water that takes 16 ounces when it's full. But fill it with marbles and it'll only hold 3 ounces. So you charge the battery and it reads full like filling up the glass of marbles, but in reality it's only holding a fraction of the charge it should.
So I'd say a battery replacement should be your first step in fixing your phone. If you want to do the job yourself, iFixit has an excellent guide giving you step by step directions on how to accomplish that. Otherwise you'll want to take it to a reputable phone repair shop.
