no lights come on with either phantom power or a proven 9 volt AC power source
can this be a symptom of a bad tube?

@paulbiere probably not. I suspect the tubes are audio tubes driving the amp but not the power. If you have no sign of life start looking at the power supply/circuit. Disassemble it using this guide Art Tube MP AMP 12 AX7A Tube Replacement and take a look for obvious damaged components, lose wiring etc. Post some good images of your boards etc. with your question. That will allow us to see what you see. Adding images to an existing question
