It is certainly possible to replace just the rear lens itself but you may have to order from like amazon or ebay or you might have luck ordering from a dedicated parts supplier. iFixit does also sell a repair kit with an OEM back glass and lens if you'd like. I'd also recommend picking up a rear camera lens protector. You should be able to find those on amazon as well.
