Wesley Krylowicz looks like the jack is soldered to the board. Use something like this guide [guide|106375] to get at it. Then measure the physical dimension of the jack and try Mouser.com or Digikey.com and see if you can match it up. It's a bit of a chore but should not be to difficult.
