PS3 or PS4, step 1 is to clean the lens(es). Qtip with IPA, gently rub on the lens(es), then use the other end to dry it off.

If that doesn't work, then you can look into replacing the laser assembly, which is generally cheaper than an entire drive.

Entire drive is an option, but you will need to either preserve your old daughterboard, or marry the new one.
