I'm not sure how you haven't found anything online, the 2011 MacBook Pros ''notoriously'' have GPU issues. The only real fix is to replace the chip. I’ll bet the issues will return even after rebaking the board, unfortinately. Software wise, you could also disable the AMD GPU and only use Intel integrated graphics, which may be the best no-cost fix for your machine.
