This is most likely caused by a battery issue. The battery is a few years old and cant retain any charge. It registers 100%  because the voltage shows high but as soon as a  load is put on it, it crashes to nominal voltage (<3.7v).  The device can be bought now for about 100 dollars, and thats also about what a battery replacement would cost.
