I found a place that will do it for $109 or $149 for both LCD and glass.  I am reluctant to post the site because I had to talk to a dumb b1tch for 30 mins (a lot of waiting), only to find out she didn't know the difference between "&" and "or".

The web site says it'll fix both LCD and glass for $109, when that is not the case, as sated above.

But I digress, I believe they are perfectly qualified to fix your problems if you want to send your Evo in.

As for me, I'm going to save $50 by ordering a screen replacement and following the instructions on this site. Just go to ebay, type in "htc evo replacement" and you'll find one (looks like base price is $52).
