The issue is either your headphone or the jack.  Try another headphone and see if that helps.  If not, then you'll need to replace the headphone jack.  Replacing the headphone jack involves removing the digitizer first.  If you haven't done this before, then you should have a repair shop do it for you as it is not easy to do this without breaking the digitizer.
