I would first observe the outside of the system for dirty clogged air vents, if it looks dirty use the PS4 teardown guide on this site to open the top of the PS4. If it's really dirty inside it may have overheated at one point. Also, look at the HDMI port of the system for damage. It may need a re-flow or an HDMI port replacement.
